Forms

The State of Texas requires clients to complete certain forms before beginning the massage. If you wish, you may fill out the forms before you come to the office and save a little time.

• One of these is required:

  • Client Intake Form or

  • Client Intake Form for Pregnant Clients
     

• This one is required only for minors:

  • Parental Release for Minors (note: even with the consent form on file, I prefer that parents remain in the therapy room while the treatment is in progress.)

• This breast massage consent form  is optional, since you can indicate consent on the back of the client intake form (above).

  Note: Breast massage is not part of a routine massage, and I will not touch your breasts unless you request lymph drainage therapy to treat breast pain. (Although males may need the same sort of therapy, the state does not require the form for males.)
